I absolutely cannot emphasize this point enough:
Before you begin the process of upgrading your CRM 4.0 organization to CRM 2011, perform an export of all of your customizations.
With an exported customizations file, you have so many more opportunities for recovery, should something go wrong during or after the upgrade.
